Annual Review Process

To provide thorough preparation, assessment and follow-up, we allocate up to five hours per year to this annual review process. We will meet with you to review the ROCC Assessment together, document progress and plan appropriately. Using insights from the ROCC, we then create a personalised Communication Action Plan (CAP), working together to plan next steps. Collaborative planning for services enables us to maximise the effectiveness of your NDIS funding and ensures every person receives the attention and care they deserve. What is the ROCC Assessment Tool?

The Roadmap of Communicative Competence (ROCC) is a tool we use to create a personal communication roadmap for each client. It’s how we check-in on their communication journey, celebrate strengths, recognise the progress made and discover new areas for growth. When we meet, we’ll talk about what’s going well, what you’ve been working on and which skills you’d like to explore next. Using information from the ROCC, we can track progress over time and create a shared understanding for everyone on the team.

What is a Communication Action Plan (CAP)?

The Communication Action Plan is a dynamic document that guides our work together towards meaningful communication goals. It’s like a fitness plan for better health, or a business plan for a new organisation. Using information from the ROCC, we outline specific communication goals and the tools, strategies and actions needed to achieve real outcomes. The CAP enables accountability for NDIS requirements, showing how each step we take aligns with broader goals for communication and self-determination.

How am I charged for the ROCC and CAP?

Please review your Two Way Street Service Agreement for full details on how we charge. We allocate up to 5 hours of time for thorough planning and accurate assessment using the ROCC and CAP. Rest assured, these hours do not need to be used all at once and will be planned in collaboration with you. We may be able to use time in current sessions and will ensure any additional time that may be required aligns with your current plan arrangements.
